Recombinant human erythropoietin
Indications/Uses
Anemia due to erythropoietin deficiency in adult chronic failure patients & in predialysis, hemodialysis, peritoneal dialysis patients & ped patients on hemodialysis; anemia in cancer patients w/ nonmyeloid malignancies who are undergoing chemotherapy treatment; anemia in HIV-infected patients on zidovudine w/ endogenous erythropoietin levels ≤500 milliunits/mL. Aids autologous blood collection in patients scheduled for elective surgery w/ hematocrit of 33-39%, who cannot produce the required blood w/o exogenous erythropoietin. Adult patients scheduled for elective surgery w/ mild to moderate anemia in which moderate blood loss is expected.
Dosage/Direction for Use
Chronic renal failure 50-100 IU/kg IV/SC 3 times/wk. Maintenance: Decrease dose by 25 IU/kg/dose. Adult predialysis patient 50 IU/kg IV/SC 3 times/wk. Maintenance: 17-33 IU/kg 3 times/wk. Acute hemodialysis patient 50 IU/kg IV/SC 3 times/wk. Maintenance: 30 & 100 IU/kg 3 times/wk. Adult peritoneal dialysis patient 50 IU/kg SC 2 times/wk. Maintenance: 25 & 50 IU/kg 2 times/wk. Ped hemodialysis patient 50 IU/kg IV 3 times/wk. Maintenance: >30 kg 30-100 IU/kg 3 times/wk, 10-30 kg 60-150 IU/kg 3 times/wk, <10 kg 74-150 IU/kg 3 times/wk. Cancer patient 150 IU/kg SC 3 times/wk. HIV-infected patient on zidovudine 100 IU/kg IV/SC 3 times/wk for 8 wk. Adult surgery patient in an autologous predonation program 600 IU/kg IV 2 times/wk for 3 wk prior to surgery. Perisurgery patient w/o autologous blood donation 600 IU/kg daily for 10 days before surgery & repeat on day of surgery, then continue for 4 days after surgery.
Contraindications
Hypersensitivity. Uncontrolled HTN. Severe coronary, peripheral arterial, carotid or cerebral vascular disease.
Special Precautions
Pregnancy & lactation.
Adverse Reactions
HTN, CVA, transient ischemic attacks, aneurysms, DVT, MI, pulmonary embolism & clotting of artificial kidney. Skin rashes, eczema, urticaria, inj site reactions, increased blood K & phosphate levels, increased BUN, creatinine & uric acid.
